-Write you life story before coming to the US (if that is the case) and in the US until the present.  It is important to emphasize problems, difficulties or circumstances that might represent extreme hardship for you -and your spouse, children, or close relatives- in the event your spouse is denied admissibility to the US.  If you have already done your Affidavit, you may use it instead of this questionnaire.  Please, email it with the other documents.
